Patient "Free No-Obligation" Consultation
or Patient Selection for Laser Vision Correction
This appointment usually lasts about 30 minutes and
includes:

Collect patient demographics to include: name,
age, address, telephone numbers, and occupation.

Discuss patient's visual needs to include:
contact lens type, prescription, and duration of use along with
frequency of prescription change and wearing time duration with
ocular tolerance.

Determine spectacle prescription to include:
monofocals, bifocals, trifocals or progressive lenses.

Interview patient with regards to refractive
needs and the type of refractive procedure that would be
appropriate to satisfy these needs.

Discuss budgetary requirements of the patient
and availability of scheduling for patient for surgery.

Present video presentation and information with
regards to refractive surgery. This should include risks,
efficacy, benefits, and limitations of surgery.

Dr. Shealy's credentials and training will be
discussed and presented.
Your
consultation may be followed up by a scheduled hour-long
baseline examination to determine further suitability.
Patient "Examination," as defined by the American Academy
of Ophthalmology in 2002
The baseline examination, which usually lasts about an hour
and includes a $175.00 charge will, in general, determine the suitability of
a candidate and should meet these criteria:

Manifest and cyclplegic refraction.

Measurement of intraocular pressure.

Slit-lamp biomicroscopy.
-
Tear film evaluation. "Dry eye testing."

Placido disc topography scans.
-
Evaluation of preoperative corneal thickness.
-
Dilated fundoscopic examination.
-
Measurement of scotopic pupil size.
An additional $175 ($350 total) will
be charged for examination if the Ocular Analyzer "Wavefront
Abberrometry" or Pentacam "Corneal Thickness" testing is necessary
for a completed evaluation.
