Consultation and Surgery on the Same Day

Many of our patients come from all across the United States and travel for many hours. For that reason, we recommend that you consider having consultation and surgery the same day for your convenience. Please feel free to download you consent form and fill it out ahead of time.

The pre-surgical consultation provides a basic orientation to refractive surgery; it's benefits, alternatives and complications. A complete examination and specialized testing to determine your candidacy will be performed.

Be sure to read the "Process & Procedure Walk-Thru" for further instructions, and so you will know what to expect.

Please register at the front desk upon arrival. Please have your consent form filled and test completed to present to our front assistant.

Travel and Accommodations

We invite you to stay at Don Hall’s GuestHouse, either before or after surgery, for your convenience since you are traveling a long distance to our office. The Guest House provides a pleasant place for you to stay during your visit to the Shealy Eye Laser Center. Don Hall’s usually runs a special rate of $59.00 for a standard room with two double beds or $69.00 for a room with a king size bed. We provide shuttle service to this location about five miles from our office or you may elect to use your own transportation to get there.

Don Hall’s GuestHouse provides a restaurant and lounge and you may also enjoy dinner for $7.00 in the GuestHouse Grill. The Chef’s select dinners include steak, prime rib, and chicken and seafood entrees. Each guest room includes complimentary coffee, ironing facility, voicemail and high-speed Internet access.

The GuestHouse is located at 1313 West Washington Center Road, Fort Wayne, Indiana 46825. Room reservations can be made by calling 1-800-348-1999 or 1-260-489-2524 Fax: (260) 489-7067. Traveling from North of Fort Wayne

Take I-69 South to exit #115/I-469 East, go 2.6 miles. Take exit #29B onto Maplecrest Road, go south 3.5 miles. Turn right (or west) on Trier Road, go 0.2 miles. Arrive the Shealy Eye Laser Center, second entrance on your left.

Traveling from West of Fort Wayne

Take US 30 East to exit #109B, I-69 North. Travel north on I-69 to exit #115/I-469, go 2.6 miles. Take exit #29 toward Maplecrest Road, turn left on Maplecrest Road, go south 3.4 miles. Turn right (or west) on Trier Road, go 0.2 miles. Arrive at the Shealy Eye Laser Center, on your left.

Traveling from South of Fort Wayne

Travel north on I-69 to exit #115/I-469, go 2.6 miles. Take exit #29 toward Maplecrest Road, turn left on Maplecrest Road, go south 3.4 miles. Turn right (or west) on Trier Road, go 0.2 miles. Arrive at the Shealy Eye Laser Center, on your left.

Alternately, you can;

From I-69 North/exit #96A unto I-469 North, or from US-27 or US-30, take the ramp unto I-469 North. Travel north, to exit #25 toward IN-37, go 0.4 miles. Turn left on Maysville Road (IN-37), go 2.3 miles. Turn left on Maplecrest Road, go 0.5 miles. Turn right (west) on Trier Road, go 0.2 miles. The Shealy Eye Laser Center is the second entrance on your left.

Traveling from East of Fort Wayne

From US 30 West near New Haven, Indiana, take the ramp onto I-469 North, go 4.8 miles. Take exit #25 toward IN-37, go 0.4 miles. Turn left on Maysville Road (IN-37), go 2.3 miles. Turn left on Maplecrest Road, go 0.5 miles. Turn right (west) on Trier Road, go 0.2 miles. The Shealy Eye Laser Center is the second entrance on your left.

From Downtown Fort Wayne

Starting at downtown near the intersection of Lafayette Street and Jefferson Boulevard, take Jefferson east toward Clay Street, go 0.5 miles. Continue on Maumee Avenue, go 0.5 miles. Turn left on South Anthony Boulevard, go 2.0 miles. Bear right on Crescent Avenue, go 0.6 miles. Turn right onto East Coliseum Boulevard (IN-930 E), go 0.2 miles. Turn left onto Trier Road, go 2.2 miles. Arrive at the Shealy Eye Laser Center on the right.
